Project Description

Background

The Living Well Center (LWC) at Washington University in St. Louis delivers interprofessional care to patients who are preparing for surgery, enhancing quality of life during and after cancer treatment, optimizing health to meet a specific physical or mental goal, or those who are interested in treating a specific condition. The LWC provides unique, tailored programs to each patient with a variety of healthcare providers contributing, including physicians, physical therapists, dieticians, psychologists and rehabilitation counselors, and aims to see patient health improvements within 90-120 days.

The Living Well Center is currently implementing a strategic planning process. The Evaluation Center collaborated with the LWC to convene staff and stakeholders for planning sessions to develop a logic model for the Center and identify key evaluation questions. As a final step in this process, the Evaluation Center conducted a series of focus groups with LWC stakeholders to learn about the implementation of the interprofessional team meeting approach. This component of the LWC is somewhat unique. Understanding how it is implemented, how team members interact and/or benefit, the organizational capacity and resources needed to implement and sustain this component of the LWC model is important to improve its alignment with broader organizational goals and objectives. Data from the focus groups was used to refine the goals and objectives for the LWC’s approach to interprofessional care.
